4 Techniques to Combat Generative AI Hallucinations and Inaccuracies

Our AI chatbot confidently told a prospect we offered services we absolutely don't provide, almost costing us a $15K deal when they discovered the truth. We implemented a strict fact-checking layer where the AI must cite specific sources from our approved knowledge base for any claim about services, pricing, or capabilities. If it can't find a citation, it says "let me connect you with our team" instead of guessing. This reduced hallucinations by 90% and actually improved customer satisfaction because people appreciate honesty over confidently wrong answers.

We were building an internal tool to help our legal team quickly summarize key clauses from long, dense contracts. The initial approach was simple: we'd feed a document to a large language model and ask it to "explain the liability clause in plain English." The problem was that the model would occasionally invent details or confidently misinterpret complex legal language. These weren't just small errors; they were subtle hallucinations that a non-expert would easily miss, which made the tool dangerously unreliable. In a legal context, "mostly accurate" is the same as "untrustworthy."

The most effective technique we found wasn't about better prompts or fine-tuning, but about fundamentally changing the model's job. Instead of asking the AI to interpret and generate a new explanation, we shifted its task to extract and label. We asked it to first locate the exact sentences defining liability, then to pull out key terms like "indemnification," "limitation," and "governing law" into a structured format. We essentially turned it from an unreliable legal analyst into a highly efficient paralegal that just finds and organizes information. The human lawyer still did the final interpretation, but their work was now accelerated by 90%.

It reminds me of training a new junior team member. You wouldn't ask them to write a complex strategy memo from memory on their first day; you'd ask them to go through three specific reports and pull out the key data points. You give them the source material and a constrained task to build their competence and your trust. It taught me that getting value from AI is often less about asking it for the final answer and more about making it the best possible assistant for the person who is actually responsible for that answer.

One of the biggest challenges with AI is dealing with hallucinations. These happen when AI confidently produces outputs that sound believable but are actually false or fabricated. I learned this the hard way when I trusted a plausible-sounding inference from an LLM without double-checking, only to find it was wrong. Since then, I've adopted a "trust, but verify" mindset.

I treat every AI response as a confident first draft, not a final answer. The most important step is to fact-check key claims manually. Even if the AI provides sources, I don't take its summary at face value. I visit the source URLs myself and research their credibility. This simple habit of verifying primary sources is the best way to catch errors and avoid being misled.

AI hallucinations aren't just small mistakes. They can seriously undermine decisions and snowball into seriously incorrect conclusions if unchecked. By staying vigilant and combining AI insights with traditional fact-checking, you get the best of both worlds: the speed of AI and the accuracy of human judgment.

During my personal research, I decided to test Google Gemini to see how it handled niche technical topics such as agent-to-agent communication. I began with what seemed like a simple fact-checking task. I pasted a short passage describing three communication protocols: A2A from Google, ANP (Agent Network Protocol), and ACP from IBM, and asked Gemini to check the accuracy of that information.

The response came back with complete confidence: "These protocols are fictional. While the concepts are valid, the specific named protocols from these companies do not exist."

The problem was that Gemini was wrong. At least one of those protocols actually exists. The model had confidently rejected true information, showing that hallucinations are not only about inventing facts but can also involve confidently denying real ones.

This experience was a perfect example of how hallucinations occur in large language models. When a model encounters a topic that is underrepresented in its training data, it tries to fill in the gaps with patterns that sound plausible. Gemini did not intend to mislead; it simply generalized from incomplete knowledge, producing something that felt correct but was not.

The best technique I have found to prevent this is to make the model verify information before it answers. I call it a "grounded response mode," which means connecting the model to a retrieval layer such as RAG or a web search and asking it to cite at least one credible source before forming a response.

Since adopting that approach, I have seen a significant reduction in hallucinations. It is a small but powerful shift that helps AI move from guessing confidently to checking consciously.
